Counselling questionnaire added to the young people centre website

The Young People Centre offers a range of youth services in central Brighton. One of these services is counselling, which young people need when they are facing tricky issues like eating disorders, anxiety or self harm.
What happens when counselling is over?
When young people who have benefited from counselling decide they can now move on and stop using this service, they are issued with a questionnaire. Until now, this questionnaire took the form of a printed sheet of paper.
How my website builder helped deliver an improved experience
The young people centre website is using my website builder, so it wasn't too difficult to add a new page introducing the questionnaire.
Anonymous counselling questionnaire to gather young people feedback
I made the counselling questionnaire in survey monkey. I chose surveymonkey because it's important to protect the anonymity of the young people who answer the questionnaire, and this survey platform makes it possible to gather feedback without requiring any login.
